Japan: share of medical adverse events by injury degree 2022

In 2022, approximately 7.8 percent of medical errors led to deaths of patients, according to the reports submitted by medical facilities to the Japan Council for Quality Health Care (JQ). Designated by the health ministry in the country, JQ conducts various third-party evaluations of healthcare institutions and provides a database of medical adverse events and near miss events with the aim of improving the healthcare system in Japan.

(2) Unapparent errors in treatment or management that resulted in the patient’s death or mental or physical disability, or required unexpected treatment, treatment to an unexpected extent, or other medical procedure (including events possibly associated with treatment or management provided; limited to unexpected events).
(3) Other than those described in (1) and (2), information conducive to the prevention of medical adverse events and their recurrence at medical institutions.
